Our forms require use of the free Adobe Reader Software – download from the Adobe website. Please print and complete these forms below prior to your first appointment.
Because of changes in the health care industry, insurance does not always pay for all care. We will assist with your insurance billing, but we cannot bill your insurance unless you furnish us with all insurance information and bring your ID card with you to your appointment. We will also assist in obtaining the pre-certifications that are required for surgery, diagnostic testing or second opinions.
We accept most insurance plans, including PPOs, some HMOs, Worker’s Compensation, and Medicare. Please check with your insurance to verify our participation in your plan prior to scheduling your appointment. Please be aware that your insurance coverage is a contract between you and your insurance provider. We are not a party to that contract. You are responsible for paying any claims that are not covered by insurance. We also ask that you pay any co-payment required by your insurance carrier upon arrival at WTMF.
If your insurance plan requires a physician referral authorization number, please bring that with you or have it called in to us ahead of time.
